HMI Summer Term:
Information for Enrolled Students

We have transitioned to online enrollment for the HMI Summer Term. Accepted families should view and sign our enrollment forms via the online enrollment portal

Enrollment forms and deposit are due three weeks after acceptance. All remaining paperwork, tuition balance, and the student account deposit are due by May 15.

Please visit our online enrollment portal to enroll electronically. Once in the portal, you will fill out the following three forms:

  1. HMI Summer Term Enrollment Contract
  2. HMI Acknowledgement & Assumption of Risk and Release and Indemnity Agreement
  3. HMI General Medical History Information Forms
