HMI Semester:
Information for Enrolled Students

Enrollment forms are completed online, accessible via your acceptance/onboarding email. 

Enrollment forms and deposit are due by April 4. All remaining paperwork is due by May 15 for fall semester students and November 1 for spring semester students. Tuition balance and the student account deposit are due June 15 for fall semester students and November 1 for spring semester students. Invoices for tuition payments will be mailed to families six weeks before the due date.

  1. HMI Semester Enrollment Agreement
  2. Acknowledgement & Assumption of Risk and Release and Indemnity Agreement
  3. Medical Authorization Form
  4. General Medical History Information
