HMI Summer Term:
Information for Enrolled Students

Accepted families should view and sign our enrollment forms via the online enrollment and onboarding portal. Login instructions will be emailed to you by the admissions team after acceptance.

Enrollment forms and tuition deposit are due three weeks after acceptance. All remaining paperwork, tuition balance, and the student account deposit are due by May 15.

Paperwork (to be completed online through your HMI portal):

Please visit our online enrollment portal to enroll electronically. Once in the portal, you will fill out the following three forms:

  1. HMI Summer Term Enrollment Contract
  2. HMI Acknowledgement & Assumption of Risk and Release and Indemnity Agreement
  3. HMI General Medical History Information Forms

Medical Form to be Completed by a Physician:

Health Care Provider Physical Form (exam to be completed by MD within 12 months of the start of the HMI Semester)
