Information for Enrolled HMI Gap Students

Accepted families should view and sign our enrollment forms via the online enrollment portal. Credentials for the portal will be emailed to you after acceptance.

HMI Gap admission is rolling. Initial enrollment forms and deposits are due by the date indicated on your acceptance letter. 

All remaining paperwork, as well as tuition and student account deposits, is due by July 15th for fall semester students and December 1st for spring semester students.

Invoices for tuition payments will be sent by email to families in advance of the due date.

Paperwork (to be completed online through your Enrollment Portal):

Please visit our online enrollment portal to enroll electronically. Login credentials are shared via email. Once in the portal, you will fill out the following three forms: 

  1. Enrollment Agreement
  2. Acknowledgement & Assumption of Risk and Release and Indemnity Agreement
  3. General Medical History Information Forms

Medical Form (to be uploaded into your Onboarding Portal):

Health Care Provider Physical Form (to be completed by a medical provider)
